Enhanced Care Management (ECM)

Is a new benefit for Medi-Cal members with the highest needs that addresses the clinical and non-clinical needs through a systematic coordination of services and comprehensive care management that is community-based, interdisciplinary, and person-centered.

If you qualify for ECM, you will be assigned a Lead Case Manager who will coordinate care and services with doctors, mental health providers, specialists, pharmacists, and social services providers to get you the right care at the right time.

Who can receive ECM Services?

To receive ECM services, you must be enrolled in a Medi-Cal managed care health plan and meet certain eligibility requirements.

• Adults and families who do not have stable housing
• Adults who are or were going to Whole Person Care services in San Diego County
• Adults with complex health issues that needed to go to the hospital or emergency department many times over the last 6 months
• Adults with serious mental health conditions or drug or alcohol abuse
• Adults at risk for institutionalization who are eligible for Long Term Care services
• Nursing Facility residents transitioning to the community
